• Each persons name, code of wellington boot choice and any additional welly boot add-ons you would like. (Max 8 names)

• List the above names in the order you would like their wellies to appear on the print from left to right and supply ages for any children so that their wellies can be sized accordingly (ages will not be printed – for sizing only)

• If you would like the wooden heart sign state the date/year or short message to go in it.
